General Description

The Supports Coordinator / Case Manager has primary responsibility for assisting eligible consumers with the development of a Person/Family Centered Plan (P/FCP) of Service. The Supports Coordinator / Case Manager works with the consumer and his/her chosen support network in developing the P/FCP to design a plan that promotes individual choice, the use of community resources, and formal and informal supports in pursuit of the individual’s desired future. The Supports Coordinator / Case Manager assists the consumer in the assignment of available agency resources to the plan. The Supports Coordinator / Case Manager ensures the monitoring of supports and services according to the plan.

In addition, the Supports Coordinator / Case Manager manages agency resources allocated through the Utilization Management System and the P/FCP. The Supports Coordinator / Case Manager provides the agency with data required for determining consumer satisfaction, treatment and service outcomes and effective, efficient utilization of agency resources. The Supports Coordinator / Case Manager is assigned to work with a specific specialty population, i.e. adults with Severe Mental Illness (SMI), elderly adults with SMI, children and youth with Severe Emotional Disturbances (SED) or adult or children consumers with Intellectual/Developmental Disabilities (I/DD), and individuals who have Substance Use Disorders. .

Minimum Qualifications
• Bachelor’s degree from an accredited program in Social Work.
• Licensed by the State of Michigan to practice as a Social Worker.

Adults with SMI
• Education, training and/or work experience in the behavioral health field with adults diagnosed with severe and persistent mental illness.

General Position Competencies:
• Ability to communicate verbally and in writing effectively.
• Ability to meet at times and locations chosen by the consumer.
• Possess or acquire ability to use computers in a clinical setting (or a non-clinical setting such as in the community or at a consumer’s home).
• Familiarity with managed care, state, federal, and private health insurance benefits.
• Basic reading and arithmetic skills are required.
• Excellent customer service skills are required.
• Attention to detail and excellent organizational skills are required.
• The ability to demonstrate absolute discretion with confidential information is required.
• Experience with Microsoft Office applications (Word, Excel, Power Point, etc.) is highly desirable.
• Experience using office equipment (including, but not limited to, desktop telephone, computer, copier, fax machine, scanner, laser printer, and desktop calculator).

Essential Duties and Responsibilities
1. Assist a consumer with developing his/her Person/Family Centered Plan to include proactive plans for future needs, and incorporate community and other external resources into the plan to meet the consumer’s long-term needs.
2. Assist the consumer in scheduling Person/Family Centered Planning meetings at a time and location convenient for the consumer and his/her chosen planning team, and act as the meeting facilitator at the consumer’s request.
3. Assist a consumer to monitor and update his/her Person/Family Centered Plan (P/FCP) and perform periodic consumer satisfaction assessments as specified (on at least an annual basis).
4. Collect data related to service and treatment outcome for the agency from contract service providers.
5. Present P/FCP to Utilization Management system for authorization and reauthorization as required.
6. Monitor contract agency treatment plans.
7. Handle consumer concerns and convey consumer complaints, grievances and appeals to the appropriate individual(s) according to LCMHA policy.
8. Perform duties and responsibilities in ways that promote goodwill, build positive relationships, preserve human dignity, and contribute positively to the LCMHA service organization.
9. Carry out additional duties as assigned by supervisor.

• Complete and submit documentation within required time frames.
• Maintain complete, detailed records consistent with Michigan Department of Health and Human Services (MDHHS) standards and agency procedures.
• Work cooperatively with program staff and across programs at LCMHA.
• Maintain positive working relationships with agencies in the community.
• Demonstrate a person centered approach to assessment and treatment.
• Attend LCMHA mandatory in-service training.
• Maintain (at own expense) licensure and/or certification required to provide professional services in Michigan.
• Participate in LCMHA Quality Improvement Program.
• Attend all required staff training as mandated by LCMHA and Michigan Department of Health and Human Services.
• Comply with all LCMHA policies and procedures.

Physical Demands and Work Environment
Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer screen and hearing and speech sufficient to communicate in person or over the telephone.

Should be able to bend, stretch, reach, and lift up to 20 pounds. An individual in this position needs to move about inside the office occasionally to access file cabinets, office machinery, etc. The work environment involves sitting most of the time in an office environment infrequently exposed to extreme temperature, noise, fumes, dust, etc.

The Supports Coordinator / Case Manager will sometimes work at off-LCMHA sites within the community (for example, at a contracted agency’s location or at a consumer’s home).

Special Position Requirements
This position requires possession of a valid U.S. driver’s license or method of transportation to travel within Lenawee County and to meetings outside of the County when requested.
