If so, we have an exciting opportunity for you to work as a Specialized Case Management Clinician!

What You Will Be Doing
Working as a Specialized Case Management Clinician you will work in collaboration with community providers and resources in order to assess and/or provide the appropriate level of care. This position will provide enhanced care coordination services for a caseload of identified high-risk patients/consumers as well as work as a team to provide short-term care coordination for individuals experiencing a mental health crisis. This candidate will work in collaboration with community providers and resources in order to assess and/or provide the appropriate level of care. This will include providing outreach services, writing and monitoring Behavior Treatment Plans and assessments, evaluating individuals for safety, and completing clinical documentation in a timely manner.

What We Require
You will have a Master’s degree in Social Work, Psychology or related field with a minimum of three years of experience in clinical settings working with the target population. Required licensure: LMSW, LLMSW, LLP, TLLP, LPS, LP, LPC.
