Seeking a Master’s level Case Manager to provide outreach services that support a growth-producing family environment and increased independent living skills for at-risk youth and families in Van Buren County.

The Clinical Case Manager will:
Use a family-centered process to assess the needs & wishes of youth and their family.
Develop a person/family centered plan.
Monitor progress related to goals.
Provide interventions to enhance or restore capacity for social and interpersonal functioning in the home, school and community.
Provide outreach crisis/clinical intervention and case management.
Link and advocate with other agencies to ensure youth receive needed health and human services.

Qualified candidates will possess:
Master’s degree in social work, psychology or counseling with state licensure.
At least 1 year experience and training in the examination, evaluation and treatment of minors and their families.
