Job Summary:
The Self Determination Coordinator coordinates the implementation of Person-Centered Planning and the principles of Self Determination in the lives of people supported by Community Living Services, Inc. (CLS) and assures that our funders’ expectations and standards are met. The Self Determination Coordinator plays a key role in the recruitment and training of Independent Supports Coordinators. The Self-Determination Coordinator’s responsibilities also include the mentoring of Independent Support Coordinators in Person-Centered Planning, budget development and authorization, community outreach, obtaining and utilizing stakeholder input, assuring quality services, and satisfaction with services while keeping the people supported by CLS and their needs the primary focus of all activities.
Qualifications and Requirements:
Minimum of Bachelor’s Degree in a Human Services area, and current, valid State of Michigan License, as required. Minimum one year experience working with people with intellectual/developmental disabilities. Meets Qualified Intellectual Disability Professional (QIDP) criteria in the State of Michigan.
Special Knowledge/Skills: Knowledge of the principles of Person-Centered Planning and Self-Determination. A clear understanding of the roles and responsibilities of the Independent Support Coordinator and the Support Circle is necessary. Must have practical experience, and demonstrated effectiveness in the development and monitoring of individual service plans and individual budgets. Good understanding of mental health functions and systems. Ability to use initiative, plan, organize, maintain systems, and meet deadlines (while paying attention to detail and maintaining a high level of accuracy). Ability to work well within a team atmosphere and maintain a positive, upbeat attitude. Desired attributes include creativity and a willingness to develop knowledge bases and share information with peers. Flexibility and a willingness to learn. Good interpersonal and negotiation skills with the ability to communicate effectively and establish positive professional relationships with both internal and external stakeholders. Proficient computer skills required, including word processing, spreadsheets, flowcharts, and use of internal and external e-mail. Knowledge of Microsoft systems preferred. Ability to accept on-call responsibilities and work adjusted hours that may include evening meetings.
