OBRA/PASRR Registered Nurse (Contingent)

(THIS IS A CONTINGENT POSITION)

INCUMBENTS WILL BE PAID PER ASSESSMENT

Under the general supervision of the Director of Integrated Care, the OBRA/PASRR Nurse Evaluator is responsible for completing Pre-Admission Screening Resident Reviews (PASSR) Level I evaluations. The Nurse Evaluator determines if a qualifying PASRR condition is present and performs physical care as appropriate.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Completes Pre-Admission Screening Resident Reviews (PASSR) Level I evaluations.
• Determines if a qualifying PASRR condition is present and performs physical care as appropriate.
• Conducts evaluations prior to nursing facility admission to confirm the presence of a disability.
• Meets with clients to conduct evaluation services and physical health care outreach in their homes, hospitals, nursing homes, long-term care settings and other types of residences.
• Performs physical health assessments and evaluations of current care.
• Completes PASRR summary for physical examination and medication history.
• Documents significant changes that signify the need for an updated evaluation.
• Completes evaluations in accordance with federal guidelines and regulations.
• Communicates all evaluation findings to clients and guardians.
• Enters relevant information into PASRR data system in accordance with federal, state and the Detroit Wayne Integrated Health Network (DWIHN) guidelines and regulations.
• Completes PASRR summaries, progress notes, nursing assessments and other clinical and statistical records.
• Records relevant information on initial and annual PASRR, Status Reviews and other treatment reviews and completes documentation within the specified time frames.
• Records services on Progress Notes and elsewhere, as indicated, in clear and concise language and in the Unit format.
• Completes nursing care documentation relevant to admission, transfer and discharge of evaluated persons within program specified timeliness.
• Reviews PASRR records, recommendations and summaries.
• Summarizes labs, physical exams, current medication, client’s needs, nursing care, medical diagnosis and treatment on the PASRR record.
• Submits completed Assessments, Progress Notes and Service Activity logs to DWIHN management.
• Manages weekly caseload to comply with Federal Regulations.
• Attends and participates in assigned committees and meetings on a monthly basis.
• Performs clinical interventions that are directed toward maintaining clients within least-restrictive settings and toward maximizing capacities for independent functioning.
• Maintains individual records, HIPAA regulations and client confidentiality.
• Completes quality assurance audits
• Performs related duties as assigned.

• Knowledge of the Michigan Mental Health Code.
• Knowledge of MDHHS policies, rules, regulations and procedures.
• Knowledge of Federal policies, rules, regulations and procedures as it relates to DWIHN.
• Knowledge of behavioral health and mental health principles and practices.
• Knowledge of the Michigan Medicaid Provider Manual.
• Knowledge of MHWIN.
• Knowledge of general nursing practices and principles.
• Knowledge of the Omnibus Budget Reconciliation Act (formerly known as the Nursing Home Reform Act of 1987) (OBRA) rules, regulations and requirements.
• Knowledge of Pre-Admission Screening and Resident Review (PASRR) guidelines.
• Knowledge of the 3877/3878 database.
• Knowledge of MHWIN.
• Knowledge of Level 1 and Level II Assessments based on the MDHHS PASRR Operation Manual Guidelines for completing a comprehensive Level II evaluation.

REQUIRED EDUCATION:

A Bachelor’s of Science in Nursing (BSN) or an Associate’s Degree in Nursing (AN/ASN) from a recognized college or university.

REQUIRED EXPERIENCE:

• Three (3) years of full-time paid professional experience as a Registered Nurse or its equivalent.

AND

• One (1) year of full-time paid professional experience as a Registered Nurse or its equivalent in a behavioral health setting performing (PASSR) Level I evaluations.

PLEASE NOTE: Experience as a Registered Nurse may be substituted for required education on a year for year basis.

REQUIRED LICENSE(S):

A Valid State of Michigan clinical licensure: RN
A Valid State of Michigan Driver’s License with a safe and acceptable driving record.

WORKING CONDITIONS:

Work is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan. This position can work remotely with supervisory approval. Currently this position is primarily a remote position.
