What You Will Be Doing
As a Nurse Care Manager, you willhave the opportunity to specialize in integrative health by coordinating patient care across the behavioral and physical health spectrum of services. You will be responsible for a caseload of high risk patients and will work as a liaison between ISK and other physical health providers to ensure your patients receive appropriate services and continuity of care. This may include triaging referrals, scheduling mental health assessments or emergency mental health services, and/or coordinating a same day psychiatric consultation for primary care doctors. You will develop an integrated health care plan for your patients and work in collaboration with other providers to ensure care plan goals are met. Additionally, you will be responsible for tracking outcomes of patients participating in your program. This is a grant funded position through April 30, 2022.

What We Require
Associate’s degree in Nursing (Bachelor’s preferred) along with a valid RN license in the State of Michigan, valid driver’s license and reliable transportation. A minimum two years’ experience in the primary care or mental health field, preferably in a community setting. Strong understanding of recovery-oriented services for individuals who have psychiatric needs, are intellectually/developmentally disabled, and/or who have substance use disorders. Intermediate computer skills, including MS Word, Excel, and Outlook. Strong experience with electronic health records and the ability to work within more than one system.

Physical Requirements/Working Conditions:
Physical Efforts– Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot.

Working Conditions– Office environment with noise from computers, copy machine, and telephones. Use of video display terminal (VDT) for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing VDT. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
