DESCRIPTION

Description of Work
A Direct Service Professional provides direct services for consumers in a community, home-based and/or office-based setting; provides skills training and development, community living supports, out-of-home habilitation and prevocational services, health and safety, and transportation services; completes consumer progress notes for face-to-face meetings and significant phone or other events; and follows up with Person-Centered Planning goals and objectives through appropriate documentation. A Direct Service Professional coordinates services for consumers appropriate to their wants and needs and the Person-Centered Plan; links consumer to agency and community resources for which they are eligible; and advocates for individuals in areas of health and safety, placement, financial, skill-building, social, recreational and family relationships. A Direct Service Professional completes documentation required by the agency, Michigan Department of Health and Human Services, Commission on Accreditation on Rehabilitation Facilities and other regulatory bodies as required for clinical staff. A Direct Service Professional performs other related duties as assigned.

Required Minimum Entrance Qualifications
A. Required Experience and Training

Be a high school graduate or have a certificate of completion of the General Educational Development (GED) test.
Have one (1) year of experience in a human service role. Please note, internships count as work experience if they are documented in the Work Experience section of the employment application.
Possess a valid driver’s license and the ability to obtain a Michigan Driver License within 30 days of employment.
Must have reliable transportation that may have to be used to carry out job duties of this classification.
B. Preferred but not Required
Bilingual candidates highly desired.
Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
Individuals in Recovery and individuals with experience in Armed Services valued.
Physical Conditions / Work Location
PHYSICAL ACTIVITIES
An employee in this class performs generally sedentary work activities requiring the occasional lifting of objects weighing up to thirty-five (35) pounds or less.

ENVIRONMENTAL CONDITIONS
A Direct Service Professional works within the Client Services offices and program location of HealthWest, with considerable travel required throughout the County by use of personal resources.

EVALUATION CONTENT
Applicants for County employment may be subject to a post-offer criminal background check, depending on the nature and assignment of the position. When such check is conducted, an offer of employment is contingent upon the results of the criminal background check. Appointees must satisfactorily complete a probationary period before the appointment will be considered permanent.

PURPOSE
The purpose of this recruitment is to establish an eligible list from which to fill present and future vacancies in this classification in the County system. For each opening, the Department Head has her/his choice of the candidates who meet the required minimum entrance qualifications. This list remains in effect for three months unless exhausted or extended.
