The Outreach Therapist is responsible for providing home-based therapy services, annual assessments, determining the appropriate treatment plan, and/or making referrals to other service providers, if deemed necessary, to consumers receiving mental health services. Must be able to demonstrate sensitivity, knowledge and use of appropriate approaches, skills and techniques which reflect an understanding and awareness of trauma and social, cultural and economic diversity of the target population served. Must actively support the goal of person centered planning and integrated care.
