Event Details
ACT team members will discuss areas that have been identified by MIFAST reviews as the most common areas needing improvement. These areas will be addressed as they relate to the ACT improvement model, Michigan Medicaid Provider Manual, the MiFAST ACT fidelity tool and the Field Guide to ACT (including SAMHSA requirements). Team members will be given the opportunity to address specific issues that they are experiencing and trends/solutions that have been noted across the state will be explored.

Objective
1. Team Makeup/roles/Team Meeting: Identify makeup of a fully staffed ACT Team, identify different roles of each team member, identify 3 core functions of the team meeting.
2. Outcomes: Be able to identify 3 reasons why outcomes are important and tools for tracking outcomes. 
3. Identify SAMHSA’s four dimensions of recovery and why engagement is an important component of these dimensions.
4. Identify the stages of change in the Transtheoretical model and how they relate to stages of treatment.
5. Person Centered Planning: Identify what a smart goal is and how to write smart goals for the various stages of change/treatment.
6. Review of difficult cases.

CEUs Social Workers: This course qualifies for a maximum of 5.5 Continuing Education hours per day. The Community Mental Health Association is an approved provider with the Michigan Social Work Continuing Education Collaborative. Approved Provider Number: MICEC 060818.
Certificate Awarded: At the conclusion of this conference, turn in your Certificate of Attendance form to the CMHAM Staff to be approved. You will turn in the top sheet & retain the bottom sheet which serves as your certificate. No other certificate will be given.

Substance Abuse Professionals: CMHA is approved by Michigan Certification Board for Addiction Professionals (MCBAP). CMHA maintains the responsibility for the program and content. Substance Abuse Professionals participating in this training may receive a maximum of 5.5 related contact hours per day. Some "Related" workshops may meet MCBAP/IC&RC educational requirements for focused topics in addiction certification domains, such as for Peer Recovery or Supervisor credentials. It is important that attendees keep a copy of the conference program brochure containing the workshop descript
National Accreditation Rules: National Accreditation rules indicate that if you are over five (5) minutes late or depart early, you forfeit your continuing education hours for the entire training. Please note that this is a National rule that CMHA must enforce or we could lose our provider status to provide continuing education hours in the future. This rule will be strictly followed.
Nursing: Continuing Education Opportunities will be available for Nursing Upon Approval
Cancellation Policy If you do not cancel and do not attend, you are still responsible for the full registration fee. Substitutions are permitted at any time. Cancellations must be received in writing at least 10 business days prior to the conference for a full refund less a $10 administrative fee. If cancellation is received less than 10 business days prior to the training, no refund will be given.
